Child & Adolescent Psychiatry Portal (CAPP)
Program Overview:
Launched in 2019, the UCSF Child & Adolescent Psychiatry Portal (CAPP) is a pediatric mental health care access program modeled after other established PMHCAPs including the Massachusetts Child Psychiatry Access Portal (MCPAP) and the state of Washington Pediatric Access Line (PAL). CAPP provides real-time, direct telephone consultation for pediatric primary care providers (PCPs) to help them manage common mental health conditions in their patients. CAPP also offers continuing medical education (CME) for PCPs, including a Project ECHO and monthly webinar series; extensive website resources for PCPs and patients/families; direct virtual psychological consultation for families; and bridge care coordination for families having difficulty accessing needed mental health resources. CAPP is also offering asynchronous virtual consultation for PCPs and school-based mental health consultation and education. While assisting PCPs in providing mental health treatment, CAPP also takes a trauma-informed, wellness-oriented approach that emphasizes prevention and wholistic care, and not just medications.

SmartCare Behavioral Healthcare Consultation Service (SmartCare BHCS)
Program Overview:
SmartCare BHCS is a well-established child psychiatry access program that has been in operation for over seven (7) years that is a pivotal component of the public San Diego County Behavioral Health Services’ expansive integrated behavioral healthcare initiative. The pediatric component of SmartCare was designed to meet the needs of pediatric primary care practices and the families they treat throughout the County. SmartCare BHCS aims to increase access to mental health care services and supports by providing pediatric PCP's education, consultation and referral options to address the needs of their patients. We have established MoUs with virtually all pediatric practices in the County that serve Medi-Cal (Medicaid) insured and uninsured youth and families and provide consultation to increase the comfort, competency and capacity of these practices in managing mild-moderate, commonly occurring behavioral health conditions. We work actively with both private practices and FQHC pediatric clinics and consult with providers and families without regard to their insurance or financial status, at no cost.
SmartCare BHCS has two core service components—one directed toward our pediatric partners in their roles as diagnosticians and treating physicians, and a second component geared to assisting parents and caregivers (and older adolescents and transition age youth) in their efforts to interface with their pediatricians and to navigate across the numerous child-serving systems (e.g., schools, child welfare, developmental services, recreational and cultural institutions) to obtain needed services.
In addition to providing OnDemand telephone and HIPAA compliant email consultations for PCPs with our psychiatric and psychosocial clinical team, we offer PCPs the opportunity to refer parents/caregivers and their children for in-depth clinical televideo consultations to assist in assessment, diagnosis, treatment planning, and guidance on resources and referrals. SmartCare BHCS communicates regularly with enrolled pediatricians and practices through the distribution of our e-newsletter that provides pragmatic information about the diagnosis and treatment of common behavioral health problems and maintain these articles on our website for access as needed. We also collaborate actively with the local AAP chapter and the UCSD Developmental Pediatric and Child/Adolescent Psychiatry clinics and training programs to support both clinical service delivery and education of trainees.
Our Parent/Caretaker Support Services are accessible on referral by family’s pediatrician and this service provides opportunity for our busy pediatric partners to obtain more detailed information about their patients’ challenges and resources with written reports that more fully discuss the youth and family’s developmental history, symptom profile and other clinically relevant information. This service also serves to provide parents/caregivers with relevant information and, as needed, referrals for supplemental behavioral health, educational and other resources to support their children’s positive developmental opportunities and resilience. This service also provides telephone consultations for parents experiencing perinatal depression and other challenges and offers education and guidance on resilience building interventions for families whose children have been exposed to adverse childhood experiences.
